PRP Dosing Question:
How do you get your platelet dose to 6 billion or higher if your patient's starting platelet count is 69?
We had a recent patient with multiple medical issues that completely eliminate surgery as an option for his symptomatic KL2/3 knee arthritis.
He has stable, chronic thrombocytopenia due to his multiple medical conditions. Platelets function fine.....there just are not many of them....
Based on our human clinical research and others (Bansal, Chu, Everts, et. al.), I do not believe in micro-dosing PRP for knee OA with low volume blood draws.
I can the best response from the most patients (with a single injection!) if platelet doses are above 5-6 billion for knee osteoarthritis....and ideally even twice that level.
It is not possible to consistently get to those desired platelet dosing levels by drawing only 15-20 cc of blood.
For this patient we drew 240cc of blood ....the first 120cc were at a platelet count of 69 and we made 6cc of PRP at a count of 546....the second 120cc were at a platelet count of 77 and we made 3cc of PRP at a count of 884.
We injected 9cc and the total platelet dose was right at 6 billion.
I was satisfied with that.
This level of personalized PRP treatment would not have been possible without a hematology machine in the office.
