  • How can you avoid orthopedic surgery and stop taking steroid shots or medications? Platelet Rich Plasma (PRP) may be the answer!

How can you avoid orthopedic surgery and stop taking steroid shots or medications? Platelet Rich Plasma (PRP) may be the answer!

Regenerative medicine is the perfect solution to help keep people active with as little pain as possible. I always have to remind these warriors in my office that their imaging findings often happen as we age...some arthritis, degenerative rotator cuffs and meniscus and labrum tears.....and that we don't necessarily need to schedule surgery because an xray or MRI shows "something". I think everyone deserves to know that these regenerative medicine treatment options are real and available from thoughtful clinicians.

  • How long can a stem cell injection last? (bone marrow concentrate)

How long can a stem cell injection last? (bone marrow concentrate)

In this video, our patient describes her outcome 4 years after getting bone marrow concentrate (stem cell) injections into both of her knees. She was told she needed joint replacement but wanted to try a regenerative medicine approach first. Her results have been spectacular...she still has some stiffness but no pain. Of course, results vary and your doctor/surgeon will give you the latest info and options at Texas Orthobiologics.

  • Florida Senator Proposes One Of The Worst Regenerative Medicine Bills Yet!

Florida Senator Proposes One Of The Worst Regenerative Medicine Bills Yet!

This is Florida Senate Bill 1768 (2025)  proposed by Jay Trumbull. The bill would remove the need for the FDA to approve the use of unapproved and unproven stem cells in orthopedics.  With exactly zero years of medical training, Mr. Trumbull has proposed a bill that contravenes the work of teams of surgeons, physicians and regulators at CBER and the FDA. He has authored a bill that would allow nearly unrestricted use of unapproved and unproven stem cells in Florida in direct opposition to federal law. With the stroke of his pen he aims to eliminate FDA oversight and responsibility to approve drugs in the USA.

Dr. Buford Says Goodbye To An Old Friend....

    *Goodbye old friend......*
At the end of this past week I had to make the difficult decision to let go of one of my oldest friends. Someone who has been on my journey as an orthopedic surgeon since 1999. From my start as a shoulder and knee surgeon.....to being named a master shoulder instructor at the learning center....to helping develop and validate new products....to getting into MSK ultrasound....and to being an early adopter of the biologics/regenerative medicine changes that are growing rapidly within orthopedics.
My friend? This green Linvatec surgical tray that was a gift from Linvatec (now Conmed) for serving as lab faculty at the 1999 San Diego Shoulder Course. I was fresh out of fellowship and heading to Dallas to set up private practice and the few things I was able to put together and keep in that tray meant the world to me. I became a Conmed consultant based off of that lab experience. I have kept and used this green Linvatec tray ....as a reminder of how many people gave me an opportunity along the way....and of how hard one must work to be responsible for others. The tray finally tapped out last week and cracked....26 years of the same unquestioning service. Not sure if there are 7 stages of grief for surgical trays but I know there is at least 1!

  • Platelet Rich Plasma Versus Steroid Injections For Knee Arthritis

Platelet Rich Plasma Versus Steroid Injections For Knee Arthritis

*The Real Hidden Conclusion In This 2025 Level 1 Study Comparing Steroid to Platelet Rich Plasma*
This month (3/2025) the Journal of Arthroplasty published a level 1 research paper comparing PRP injections to corticosteroid injections for patients with KL2/3 knee osteoarthritis. The authors' conclusion? Steroid injection gives better pain relief than PRP injection at 6 weeks and BOTH steroid and PRP were not effective at 3 months. On initial review, a conclusion that steroid might be better for pain relief is actually not that surprising since the benefits of PRP continue to improve over 3 months. This would be like a level 1 paper concluding that 2% lidocaine gives better pain relief than corticosteroid at 15 minute followup.

  • Have you heard about exosomes? Here is the latest book chapter and update on using exosomes in the USA!

Have you heard about exosomes? Here is the latest book chapter and update on using exosomes in the USA!

Mesenchymal stem cell (MSC) exosomes are small, extracellular vesicles that mediate intercellular communication.
MSC exosomes may be a treatment option for patients with orthopedic conditions like knee osteoarthritis but currently there are scientific, regulatory and manufacturing hurdles restricting exosome use in the United States.
Food and Drug Administration (FDA) regulates exosomes as a drug, and since there is no current FDA approval, clinicians are restricted from using exosomes in every day medical practice in the USA.

Tips for getting the best bone marrow concentrate (Stem Cell Injection!)

    *Regenerative medicine fact*
Still believe that an open ended Jamshidi needle leads to peripheral blood dilution?
Well, It does NOT if you use good technique:
    1. Use smaller syringes (ie. 10cc)
    2. Use smaller volume draws per syringe...6-8cc in a 10cc syringe for example
    3. Draw from multiple depths AND/OR from multiple cortical entry points.
I draw from multiple depths...from superficial to deep... and just one entry!
